Suppressors Form 4 not complete

I bought a new suppressor last week. The dealer filled out the form 4 for me but in box 3a (transferor's name and address) they only filled out their name and street name leaving off the city, state, and zip. Is it ok for me to just write in the city, state, and zip? Any help would be greatly appreciated.

I would call the dealer with that question. He should know. Bottom line is that the complete address is supposed to be there.

I called the dealer and he said it was ok to write in city, state, and zip code. Thanks for the help. I hope this will help out others if they come across the same problem.
