Years ago I saw that happen at a house in hunt’s point Seattle
We were tying up a rebar mat with risers for an elevator and a framer ignored the warning signs and walk across a board we spanned above to help us stage mat layers.
Dude stepped on the board and fell down luckily missing all the risers and not impaling himself!
