I'm wanting a 6.5cm chamber, that has a slightly shorter free bore than the typical saami spec reamers out there. Do I have to order a custom reamer, or does the smith just not plunge the reamer in so deep to get that shorter freebore. Anyone know?

Do I have to order a custom reamer,
Generally, yes. Although there's probably a few variants out there already, so it's not like you need to come up with all the specs yourself.

or does the smith just not plunge the reamer in so deep to get that shorter freebore.
Only way that'd work is if the gunsmith was using a zero freebore chamber reamer, and a separate throating reamer. Not unusual in some cartridges / situations... but I'd guess somewhat uncommon on something like a 6.5CM.

Here's something that might be of interest to you:

Bear in mind, this is in no way an endorsement of that particular barrel company; I have zero first-hand experience with them. But it does cover some options as far as different freebores in the 6.5CM, and I'm assuming that if they're offering them, someone else is making the appropriate reamers. Sometimes they're in-house/proprietary designs that the reamer maker won't release without permission, ya never know til you ask. Or maybe they make a pre-fit barrel for your action with the throat that you want, and it's an easy-peasy solution.

There are quite a few shops that offer shorter freebore chambers.

Chase Fisher @FisherT&C is an excellent and affordable smith with quick turn around times and he has a .150" freebore 6.5CM reamer (vice the SAAMI free bore of .199" which he also offers)

I had him spin a CDG prefit for me....I haven't put that rifle together yet but on bore scope examination the work looked excellent and I'm happy with it. I have two personal friends who have also sent Chase barrels (which they are currently shooting) and they have been very happy with his work also.

I wanted a shorter free bore to be able to get closer to the lands if that's what the barrel likes. With the .199" CM free bore and Berger 140 HT I could only get to .070-.080" on a new barrel and stay inside of mag length...so, shorter free bore for this barrel.

Also, my thoughts were informed by the Preferred Barrel study linked by @memilanuk

    Hope this is minimally helpful.
