I am sure there ARE other reasons, but they are less common. Sounds like out of spec FCG parts. Hard to be more specific than that without having it in hand.

Check to make sure the points of contact on your disconnector, trigger, hammer and sear aren't worn.

Depending on if this is happening with the trigger depressed or released.

Depressed = disconnector issue
Released = Sear issue

Also try a new disconnector spring.

Has anyone done a "trigger job" for you? Removal of the "hardened" material in the hammer or sear will cause these surfaces to wear quicker.

Check the notch on the bottom of the hammer or the sear at the front of the trigger assembly for wear.

That's the most likely culprit. Also verify your trigger spring is in working order and installed correctly

My AR10 did this a couple weeks ago. My problem was the lower trigger pin backed out a bit, and something happend and it double tapped on me. It stopped after the second round, but my finger was also off the trigger. Ended up being an out of spec pin. I have an IRA trigger, and my local smith mixed up the pins when I had him swap my LPK from the IRA lower to my Armalite.

The times I have seen it happen were usually traced to a faulty disconnector. I've also seen it happen to a handloader who didn't trim his brass. After brass was trimmed it didn't happen again.
