Well.....the bullets are "keyholing" which could be caused from a muzzle brake being misalligned or a suppressor baffle strike, something is altering the bullets flight path or the bullet is just not stabilizing. This goes way beyond basic marksmanship and its not you making groups open up tlike that.

Does it have any type of muzzle device?

Dont see any brass shavings or any debris for that matter, so the bullet is hitting the brake which is causing the eratic shooting pattern?

Also, forgive my ignorance but will shooting like this cause any damage to the gun/barrel?

and how do I take the brake off?

Take the brake off and retry it, looks like someone was launching bullets at the target with a slingshot or someone was poking your target with a tire iron. At that distance definately something altering the bullets path.
