Man thats a pretty big thing to miss, plus the numerous dents in it, and the cut material in a few of the slots still hanging on..

Im hoping this isnt normal for PRI...always thought their stuff carried a reputation of being top notch.

Yeah, every PRI piece I've ever used is top notch. But no company does everything perfectly perfect every day.
Have you owned the older design carbon fiber handguards or one of these newer ones too? I was skeptical about this newer octagonal design as it doesnt really follow the same specs as the older SPR handguards.

Basically couldnt find any reviews on them and figured I'd take a shot on it since the brand is well-known.

I only have their tube style handguard on my Mk12 Mod 0 as well as their M84 GasBuster charging handle, and some other odds/ends. Reference the handguard, it's top notch.

Seems like a cosmetic issue - not a durability issue to me, but I wouldn't be pleased with that as a brand new product. I would contact PRI to see what they say. While I don't think this necessarily would be unsafe to use, I don't think it's up to their typical quality standards and should probably be replaced. Hopefully they take care of it for you.

I got the replacement pretty quickly. It was delivered today. Finish on the cuts is higher quality, however, the barrel nut and handguard hardware are odd to say the least.

It leaves about a quarter inch gap of exposed barrel nut (the black under the tan)

installed it looks even worse:

I checked both the replacement handguard I got and the original, and the barrel nut and jam nut + handguard both lock in place with that noticeable gap..
