Indent in brass with hard extraction, new barrel

Impact 737 with bartlein barrel, factory hornady tap 147 6.5creedmoor.
New-ish barrel with exactly 30 rounds down the pipe. Extraction issue and indent began roughly 15-16 rounds in.
No borescope, looked down the action and didn't see anything obvious. I can pull the barrel for inspection if needed, any ideas?

Clearly there’s a defect in the chamber. Address it with the entity who chambered it.
