LR 308 Bolt lug wear, front of lug?

So I had my LR 308 apart for cleaning the other day and noticed some significant wear/ burs on the front of 3 lugs on my bolt (top two lugs, and 8:00 log when facing the bolt). Looking at the barrel extension there are matching wear marks on the barrel extension. This barrel only has about 700 rounds on it and the bolt has several thousand (5000 about) on it. Specs of the rifle are as follows:
DPMS Upper and Lower,
DPMS BCG
Blackhole Weaponry barrel
Troy handguard
Superlative arms adjustable gas block
KAK heavy carbine buffer
JP tuned/polished 308 buffer spring

My load that I have been shooting in this rifle has been 42.3gr IMR 4064, in a LC LR case, CCI primer, and a 175gr Sierra Matchking

Does anyone know what may have caused this wear? A buddy of mine who has been in the firearm business for a long time said that he has seen this caused by barrel extensions that were not indexed correctly. But wouldn't this mean that the pin in the barrel extension was drilled off of top dead center? Any thoughts on what I need to replace other than bolt and barrel? Upper receiver too? Thanks for any help!
