Metal fragments in bolt

I found these little metal pieces inside my bolt today (right side of picture). Any idea what they could be?

Also, the spring/bumper came off the extractor. I can put it back in but it seems looser than in other rifles.

I haven't noticed any pierced primers, but I haven't been inspecting every round. I'm running factory ammo - Win 125 FMJ. Velocity is averaging 2840 out of a 22" bbl.
Firing pin looks ok. I have about 100 rounds through it and it shoots great - sub MOA.
 
Here's a picture of the bolt face.

It is not a light weight carrier. I have a heavy buffer and the timing seems to be good. Ejecting at 3:00 and no ejector swipes.

@tomme - what are you noticing that indicates clocked barrel extension? And how does one fix that? I know the barrel was properly torqued when installed.

FWIW, your photo shows a RISE Armament BCG... not a Rainer Arms one.

It is possible the over pressure ammo helped produce the bolt lug battering.

I wouldn't use that specific ammo anymore.

I had some Fed. 120gr OTM that fully pierced primers as well ( early production lot )

And "Blanked primers" is the more appropriate term. Thank you Tokay444

Do you have the means to measure your FP tip size, and the bolt face FP hole ( aperture ) ?

Sorry if I didn't notice, has this only happened with the Winc. 125gr ?

Any pics of fired cases ? Specifically the Blanked primer ones ?
