Happens. MUCH less common with modern primers. Good reason to use a primer flipper and look at both sides.

not really unusual, especially if you are using primer filler machines, etc. get a loose one or two and out they pop. Just need to watch out for them.

No machine used,found it on the flipper tray.
I started reloading about 1970,this is the first missing anvil I have seen.
