scope levles

You need one - its not an issue of "form" but mechanically setting ther rifle properly. A canted rifle will affect your windage results at distance.

Some mount on the scope base, some on the scope, others are integrated in the top half of the ring. It's all personal prefrence on how you want to look at it when behind the scope

I use the fixed USO.

Excellent point. Levels are devastatingly important past 500 - 600 yards with wind. For on scope levels, Flatlineops.com is arguably the best in the industry and their customer service is the best I've ever seen.

As far as scope mounts, Sphur makes the most robust, bomb proof mounts on the planet. Expensive but worth it if you believe in quality.
 
I thought they were just a gadget till i started missing windage calls oddly at 600 one day. Thought I was leveled up but when i stood up and looked at my rifle fro behind it was clearly crooked. Ha, felt level but the bullets didnt lie. I use one now
