Dual Lever Modular HD mount from American Defense. It fits a lot of AGOG's and Sightmark Wraith night vision scopes. I used it on my Halo LR with great success. I did have to open up one of the holes a bit to fit the Halo (see pics) but it wasn't much and will not effect the function. Comes with box and everything from the factory.
