Tighten the 3 hex/Allen screws once you put the base in. I tighten the 3 screws to 15 in lbs. Hasn't backed out.
You need the Apex 2 adapter, not sure from pic if that is what you have but like Nite said, just tighten the 3 screws on bottom.
The big screw is just to hold the hook under the tripod to hang shit.
