The most reliable data we have at this point is a small randomized trial that showed no effect. There are also two French studies that have gotten a lot of attention, but have extremely serious flaws and cannot tell us much.

The states that restricted this did so to prevent people from hoarding it for outpatient use and thereby preventing lupus and RA patients from being able to get it. That was not unreasonable. No one is restricting it from being used in inpatients, although it is really foolish and unethical to do so outside of a clinical trial. In the end we will likely find that patients were being harmed by it and not helped.

Aggressive testing, contact tracing and isolation of positives, social distancing, wearing masks in public...the usual things that work. Note they used a ton of HCQ and other antivirals in Italy and Spain, didn't seem to do much for their mortality.

Isn't one of the side effects liver toxicity, especially when used in combination with zithromax?
Seems to me this would be a serious problem if a person's liver was compromised, and isn't it possible it could result in permanent liver damage to a healthy liver?

The biggest concern with that combo would be the potential for QT prolongation that can lead to a fatal arrhythmia. This would be undetectable without continuous telemetry. This risk is also compounded when patients are on certain cardiac meds and/or neuro/psych meds.

Do you literally live under a fucking rock? There’s videos of people who have been given the combo praising how well it worked for them. There’s a doctor in NY that has successfully treated hundreds of people that not only says it works but even severe cases has breathing restored in just hours... but yeah, there’s no credible evidence.

Yes, thirdhand reports of individual patients aren't credible evidence. Randomized controlled trials are needed. Indeed one has been done and predictably shows no effect. I made a very detailed post last night where I explained this thoroughly. Perhaps you should read it. Sometimes subject matter knowledge is important.

What training do you have in medicine, or evaluating the effects of drugs? If you do not have such knowledge, why do you think it wouldn't be necessary?

In case anyone reasonable is reading this, please know that it's not easy to know if a medical therapy works. You cannot just give it to a bunch of patients and say to yourself "oh, they all did fine." That's how you get bloodletting. You need a control group so that you can compare how patents did without the drug, because usually the differences aren't huge. You need the groups to be randomized, because if you give your drug to a bunch of patients who are not as sick, they will do better anyway and then it will look like the drug worked when it didn't. And you need the groups to be randomized because otherwise the placebo effect can throw off your analysis.

There are countless examples of people treating patients with drugs or other therapies, convincing themselves that they worked, and then finding they had no effect or were harmful when properly tested in a trial. It's not a matter of "letting people die.' At the moment, all the information we have shows that it is more likely we will harm patients than help them by giving them hydroxychloroquine.

If you don't follow all of that, please also remember that hydroxychloroquine along with many other medications have been used already, in Wuhan, in Lombardy, in Spain, and we are still seeing high mortality. Clearly they are not any kind of miracle cure.

Besides all the people in the U.S. it has already saved and the positive results we have received, there are no less than 20 other countries that have also conducted studies and shared that information with the FDA. It works period. It has kept a doctor in NY from catching the virus even though he treats 100's of patients.

There are millions of doses of this medication already. No one is going to miss out on there doses who have arthritis or whatever else they are taking it for. Side effects at these reduced dosages seem minor from reports.

From a flip side perspective, the orders those governors made specifically restricted doctors from prescribing the meds on an outpatient basis for possible, but unconfirmed COVID19 patients with mild/moderate symptoms.

Saving the drugs from Hoarding by doctors offices and mainly keeping them to be used for those that are admitted to hospitals and in need of them more than the less sick folk.

The biggest mistake so far is the wide promotion of this combo as a cure when, as you point out, (1) the evidence to date of efficacy is at best anecdotal and (2) there are very few true antivirals.

Maybe the combo does allow for a reduction in symptoms that shorten duration or severity of the condition. Hard to say when the only reports have come out in a rushed manner from folks besieged in countries swimming in the disease and the attending desperation.

Again, cure is the wrong word. Try:

I can fully understand if they are passing the statutes to combat hoarding, as there have been numerous cases of doctors cranking out prescriptions for themselves, their families, and other people on a cash basis, which seriously disrupts the access of these drugs for those suffering from lupus and other ailments. However, some of these governors have way overstepped the line to include in their own wording specifically forbidding the use of these drugs on confirmed COVID-19 cases, which is why they are drawing a shit ton of public ire.

That said, though, treatment regimen involving this drug combination should be used in a monitored hospital setting. Quinolines are known to have a lethal dosage rate that is very close to the effective dosage, sometimes so close that the lines are blurred. We cannot have patients mixing them by themselves. With such medications, bloodwork usually have to be carried out first and then the patients kept under close electronic and lab monitoring to make sure that their bodies can handle it, at the doses dictated by the results of these lab tests.
