Clamp on vs thread on

I have a brake on an unthreaded rifle using a Witt Machine clamp on adapter.

The brake and the adapter both work exactly as they should. Zero alignment and loosening problems.

Looks a bit goofy but IDGAF.

I remember the CA AWB and the resultant CA Compliant clamp on muzzle brakes in lieu of a threaded barrel and flash suppressors. Some of the clamp on brakes flew off during the course of firing.

That is my biggest concern but can't find anything other than anecdotal evidence of it.

Did you witness this firsthand or just hear about it?
 
In my experience I would say the most important thing of clamp-on systems is the precision with which they are made.
A clamp that covers every micro-inch of the surface should work perfectly.

Correct. That's why I use a clamp-on adapter from Witt Machine. You measure your barrel OD in two places per their directions and they'll bore that taper in the adapter. Combine that taper with the large contact surface area and the ten clamp bolts and the Witt adapter is going nowhere if you install it correctly. https://www.wittmachine.net/custom-clamp-on-threaded-adapter.html

I'd rather use an adapter that has 5/8-24 threads over a clamp-on brake because I can use any standard muzzle brake instead of being limited to a few choices.
