The respected Craig Douglas/"SouthNarc" of ShivWorks has done a couple of videos very recently about entry "gaps" into duty holsters that are allowing triggers to be pulled on properly holstered weapons. Jared Reston, a rep from Safariland, is in the first vid and he goes over some improvements with Safariland duty holsters designed to mitigate the issues:



 
One consistent observation seems to be that physically narrower WMLs allow for narrower holster openings, which reduces the sizes of gaps.
