Like the title says, other than outer dimensional differences does it really matter which one you have? Or is it just a "mines mil-spec and yours isn't" kinda thing. Thanks in advance for any input.

Does not matter. You just need to know if you are purchasing a MOE or CTR to put on an existing tube. If you are building from scratch, just match up your stock to tube.

Practically speaking, probably not for most people . . . but the milspec tubes are often (not always) made from stronger materials and to a more rigorous standard.
Some stocks are only available in milspec diameters.
