Gunsmithing Scope mounting procedure?

I have had 2 scopes come loose on me recently. Both set in Nightforce rings. One on a Nightforce Picatinney rail. The other on cheap bases. I don't like Loc-tite but I do use it on the base screws. I have determined that the cheap bases came loose due to my brother not using Loc-tite and on the other scope I have not determined what happened. All screws were tight but I had constant POI shift. I thought the scope (Luepold Mark 4) had failed and mounted a friends Nightforce which was already in Barret rings. It shot well, so, not a rifle problem. I then re-mounted the Leupold and it too shot well then! Obviously something was loose originally. While the Leupold was off I undid the rings and inspected them. I found what appeared to be fine dust on the bearing surfac between the scope tube and ring. This led me to believe the scope was sliding in the rings even though the screws were tight and while mounted, no movement was apparent. (wear on the scope tube)

Long story but my questions are:

Should I loc-tite the ring screws? Blue Loc-tite?
Is there a torque spec for NF base screws, ring mounting nuts, and ring screws?
Should I have the rings lapped?

Obviously I'm doing something wrong so I've turned to you guys to set me straight.

I have run 60 in/lbs on the base screws and cross bolt nuts. 15 in/lbs on the rings screws. I only loctite the base screws and have never had ring scres come loose (except that time I drove 40 mph down washboard roads hunting antelop in WY).

Lapping isn't necessary but is beneficial if you have some high spots. I don't think it is part of your problem but wouldn't hurt if you are starting from scratch.

Put some rosin (powdered like baseball pitchers use) in the rings before installing the scope. Torque the screws to specification and put 1/2 drop green locktite on the screw heads. Clean out the recess before it sets.
