Last week, there was the entry in the Cook County, Ill., medical-examiner ledger: “Complications of opiate (probably heroin) toxicity. Novel corona (COVID-19) virus, end stage renal disease, hypertension, ACCIDENT.”

It is worth risking side effects if a drug provides some other benefit. That’s why we give hydroxychloroquine for malaria, because it works and that offsets the risks. That’s why we give chemotherapy for cancer, and anticoagulants for atrial fibrillation, even though those medications are pretty dangerous. We wouldn’t want to do that if there was no benefit.

There have been thousands of compounds with in vitro data showing activity against viruses, nearly all of which failed when tested in real patients. That is why you have to do the trials. Again, it can be worth it to accept some risks even for prophylaxis, if the drug works for prophylaxis (and there is data that HCQ does for malaria).
