Maybe dont fire it just yet. But she has no idea, she didnt even look it up after the fact when she made the video. Its reckless.

I’m thinking preceding round was a squib load and noob didn’t understand what was going on. Rack, tap, re-engage....KABOOM! Either that or cheap as shit factory or in house reload was double charged. I really liked the “this gun is not in shooting condition”...

Then no one could be bothered to show us into the chamber or the remaining pieces of the case.
From the 2.5 seconds we actually saw the chamber (not on purpose) you can see the case head has left a 'ring' of brass around the diameter of the chamber. She mentions the barrel has a slight bulge as well.
Those two things make me lean towards squib and 2nd round into the squib causing the chamber/barrel pressure to basically go backwards as the path of least resistance was the brass casing. The chamber brass ring makes me think that as well as it signifies the bolt had to be closed and then blown backwards with such force, the case left the damn case head behind after basically a fire jet of pressure blew through the middle of it. Case head separations/overcharged cases generally dont bulge a barrel from anything Ive seen.
The only thing I know of that can line all of these up is a bullet in the barrel. For a new shooter, they generally dont know WTF a squib is. When I have people start shooting I always tell them that when you shoot and a round going off sounds/feels totally different than the others, STOP.

I'd agree 100% it's amazing how many fuckin people are "Instructors" or teach a tactical class these days. Bunch of 5.11 shit, check... Drop leg holster, check... Gunfighter belt, check... Upload to YouTube, check... You now have all the knowledge and skills as a Navy Seal, please go forth and teach others...

Well yeah, the outer diameter of the case head is still fused to the outside of the chamber.
I think it was caused by a squib.
The bulged barrel mention from her was what made me think its a case head separation caused by a 2nd round into the back of a squib, over a case head separation due to a shitty/bad round.
They also didn't seem to look in the barrel.
Possible little to no powder in a reload... Primer pushed the bullet an inch down the barrel and the second round was "normal" and boom!!
Local ‘A Girl and a Gun’ instructor was observed to have a ND during a gun safety demo and never addressed it during class because no one other than another experienced shooter caught it. Mentioned it to her after class was done and got a ‘well it was pointed down range’ and move on. Not exactly the message you want to send to new shooters of any variety. Mandatory DQ in any match I’ve shot in. Shit happens, no doubt, but own it when it does, and call yourself very lucky no one injured in the process.
I don't see the problem... doesn't look like she's out there pretending to be some tacticool trainer, just teaching regular folks basic pistol. (Seems like)
I didn't see anything objectionable to her description of the circumstances, she articulated it well enough. Seems some are picking words to get a perspective they want. I'd much rather hear someone say their thoughts and not make immediate accusations than the fuckwits that going flinging their 'knowledge' around like a monkey throws shit.
I don't know anything about this girl/woman/whatever other than this video. From just this video alone, I don't see the problem with her teaching a pistol class. Some folks have an awful big head about firearms. Don't be dickweasels -- the more people are introduced to firearms the better.
