I'd like to know the back story to this. What a catastrophic failure. Could have very well lost fingers, hand, forearm, FACE. Wholly shit. We've seen some bad ones I know, and I've had a friend who blew up his 300 RUM and tore his face and forearm up pretty good, but this is next level disaster.

For clarification, I was referring to a 16 being inserted into the chamber of a 12, and it will fall to the bottom of the forcing cone. A 12 can then be inserted into the chamber, and the action will close.
If the trigger is pulled, then the 12 will go off, and be completely obstructed by the 16. Add to that, the fact that the 16 will most likely go off too, considering the primer being hit by the charge from the 12. Dual grenade in one barrel. (ish)
sheer catastrophicness.
