That's actually a misconception.
Handicapped parking is quite often well away from the doors, the emphasis in design of parking lots and placement of handicapped spaces being the accommodation of the wheel chair-bound in adaptive motor vehicles. As long as there's a cut in the sidewalk, it doesn't matter how far out they are, because they can roll. Besides, they need extra space for their ramps and lifts.
Meanwhile, people who have limited mobility and/or strength/energy but who aren't yet in a 'chair, gimping around on crutches, walkers, and canes, are forced to deal with the results of this oversight. You don't "get it" until you wind up in such circumstances.
Philly sounds like a rough place. Around here all the handicap parking are closest ones to the door and are usually 50% wider. Also the state will issue handicap permit either temp or permanent if a mechanical device is required for mobility (includes crutches, cane, walker).

Attended the NRA annual convention yesterday in Atlanta at the GWCC. Great show, and I can't remember the last time I was around that many people who were so polite, friendly, and considerate of each other.
