Had to medevac a guy back to Germany to a burn unit when one of those flares he was working on went off in his lap. Guess he was using the wrong tool and set it off. Guy was a mess. Another guy was walking out the door of the shop at the time and the flash burned him as well but not nearly as bad.

Similar incident in 95 or 96 (think it was out west). I wasn't there, but as I recall from the investigation, kid cut the barrier material with a knife or scissors. Supposedly,the flares create a gas when stored in a sealed container and they suspected a spark ignited.

I had IDed the gas issue about ten years back when I opened a M2 ammo can and the plastic caps started popping out of the squib ports. The explosive safety expert said it was rapid temperature changes.
 
I so wanted to do one of my fuel pump replacements that way. The straps which held in the fuel tank had the bolts which secured them up in an almost unreachable spot. I have never understood why ALL truck manufacturers don't have an access plate over the pump.
Would increase the cost too much... After all on an $80k truck there is no profit... LOL
