I dunno.... Bad trigger discipline... especially if he tightens his belt the wrong way.. or attempts to "draw" and forgets...
Similar incident in 95 or 96 (think it was out west). I wasn't there, but as I recall from the investigation, kid cut the barrier material with a knife or scissors. Supposedly,the flares create a gas when stored in a sealed container and they suspected a spark ignited.
I had IDed the gas issue about ten years back when I opened a M2 ammo can and the plastic caps started popping out of the squib ports. The explosive safety expert said it was rapid temperature changes.
