So this is probably the closest NASA came to losing a shuttle.... without losing a shuttle.

Once again corner cutting and trying to get out cheap.

They got damned lucky!

This just crossed my radar screen today... and I had no idea. Long gone by then. But... damn. When will these retards learn that stepping over dollars to pick up nickels is fucking retarded?

A good example of why redundancy and a bit of extra performance margin are useful measures in safety- and mission-critical systems, even when the analysis suggests that the odds of failure are "acceptably low".
