At a place called "Wild West City" in Byram NJ, one of the "actors" accidentally loaded real bullets in his gun instead of blanks. One guy got shot in the forehead and was partially paralyzed. Thank God it was a 22 and not something bigger. They definitely made some policy changes after that.
Two years ago a guy was shot and paralyzed when someone else brought a real gun into a SIM range during a tactical lesson. It was a group that "leased" the facility for the day so the normal checks done by the facility operators didn't take place. Happened in Stroudsburg PA, which is right near me. I had considered checking the place out until this happened.

You'd have to think they were lined up in front of him giving him some direction on pulling the gun and firing (quick draw stye) or something and he only shot once. I have a hard time believing he shot someone right in front of him and kept shooting. Unless, as someone already pointed out, he was throwing a liberal actor hissy fit and couldn't find a remote to shove up his own ass and started firing at everyone instead.
So, another thing I learned from the Brandon Lee fiasco is that modern movie scenes involving guns are (supposed to be) shot such that it appears that the firearms are pointed at the targets, but are actually pointed wide, and not at someone. Redundant safety measures. Kind of like, so long as you follow the first rule of firearm safety you’ll never have a problem, but we have 2, 3, and 4 for good measure. Several things went wrong for someone to get shot on a movie set. There’ll be plenty of culpability to go around.
much the same rule that many historical reenactors follow....when they skirmish, they try not to actually aim their weapons at someone, and usually aim wide/ high...just in case.

My guess is single shot throwing debris like a shotgun or they were staging a scene where he rapidly fires multiple shots but before anyone realized the gun was shooting debris a few were popped off.

It's possible for affect, he was shooting "into" the camera in a straight on angle.. Those two people would generally be behind/next to the camera.
This is simple, the gun should be taken to a range by qualified individual and test fired with live ammunition if it fires along the line of the sites meaning it’s accurate then he needs to be prosecuted and sent to prison because that means he was aiming at that producer, if it shoots 6 feet to the right 3 feet to the left something like that then it was an accident

If there was a live round in there, how did it get there? Looks like if they wanted to show the "lead" in the cylinders of the revolver they'd have a cartridge without primer and just the lead crimped. They could even improve on that and have the base of cartridge painted green or something signifying "good to use." All sorts of things they could do to help prevent stupidity (though one cannot fix stupid).
